AGREEMENT | | 1. | One animal per application. |

| 2. | Minimum age is 6 months. |

| 3. | Vaccinations must be current. |
|---|
| 4. | Application expires 30 days after date of approval. |

| 5. | Extra expenses over the basic spay / neuter will be the responsibility of the owner.

| 6. | You must make the appointment and verify the cost of procedure with veterinarian.
|
|---|
| 7. | DO NOT TAKE the animal in for the procedure until the application has been approved. |
|---|
| 8. | You must take the APPROVED application into the Vet on the day of the procedure.
|
|---|
| 9. | Your half of the cost of the procedure plus any extra expenses must be PAID in FULL on the day of the procedure. WFCHS will pay 50% of the cost of the basic spay / neuter directly to the veterinarian.
